If you pick a time online, you’ll receive a text with a link to complete your registration. If you walk in, our team will help you get started and send the link to your phone. You can enter your information and take pictures of your photo ID and insurance card—without printing or carrying a stack of forms.

We’ve kept the process simple and ask only for the information needed for your visit.
